It may cause life-threatening hemorrhage and cardiac tamponade. Penetrating cardiac injuries caused by firearms and penetrating instruments have high mortality rates. Many patients die before reaching the hospital. For those who reach the hospital, correct diagnosis, transportation time to the operating room, surgical patient management, and localization of the injured area determine mortality. In this article, we presented a 46-year-old male patient with internal mammary artery injury caused by a stab wound, which resulted in cardiac tamponade, and its successful treated with median sternotomy. Keywords : Left internal mammary artery injury, penetrating cardiac injuries, traumatic cardiac tamponade
